Now, I have been working on a missing pieces of my family puzzle which has led me to learn even a few new things I did not know about.

I have been trying to work on:

Josephine Byerly Treadway
B: 5 May 1857 in Pittsburgh, Allegheny County, Pennsylvania, USA
D: 13 Nov 1904 in Brooklyn, Kings County, New York, USA

I have her death info, which was passed down to me.

The date of death is also verified by the New York, New York, Death Index, 1862-1948 on ancestry.com.

Now, I could order the death certificate from NYC Department of Records:

Cost: $15.00

Then, after posting a message on the New York Genealogy Network facebook page.

I leaned more about NYC death certificates from info on Stephen P Morse's website.

I even emailed Steve Morse.

Dear Mr Morse:

How successful are people who use your guide in obtaining NYC death certificates
through the FamilySearch photoduplication services?

Would you please, let me know.

"100% successful, assuming that they have the correct film number, certificate number, names, and date. " -- Steve Morse.

Here I typed in Josephine's info.

I click on search IGG
What is IGG? It is the Italian Genealogical Group

The Italian Genealogical Group gives you the certificate number.
This is important.

Now, going back to Stephen P Morse's site, I use the information to look up the information for the FILM NUMBERS FOR INDEXES TO CERTIFICATES, and FILM NUMBERS FOR CERTIFICATES.

Once you know that information, you should be able to, as suggested by Stephen P Morse; obtain a copy of the NYC death certificate using photoduplication. Saving $15.00

Here is how I filled in the form using the information.

I submitted a request for Jospehine Byerly Treadway's Death Certificate through photoduplication.
I will keep you posted of the outcome.

But from my message on the New York Genealogy Network group facebook page, I was indeed fortunate enough. As another user looked up and supplied the information from Josephine Treadway's death certificate. This is what I needed for verification that the information handed down to me was correct, and that I had the right person.

It gave her place of burial, and I was able to finally put her to "Rest in Peace" on find a grave.

One simple phone call to Cypress Hills Cemetery, and her plot information is: Section: 15; Locust Grove; Grave: 11428; Division 2; Block 30

Someone had James Treadway "Taylor" (Josephine's husband) death certificate on ancestry.com.

James R Taylor

Results from my email to All Faiths Cemetery:

"Our records show that James Taylor was buried on May 11, 1935 in Public Lot 28 Map 3/a Row 68 Grave 72.
In the same grave May Taylor, age 85, was buried on March 22, 1961.

NOTE: "WE WILL LIMIT YOUR REQUEST TO FIVE (5) REQUESTS PER MONTH.  EACH REQUEST CAN CONTAIN ONLY ONE (1) IMAGE.  Requests are processed in the order they are received. Allow 4 to 6 weeks for delivery. Please do not re-submit your request a second time if you do not hear from us immediately."

The photoduplication runs on a 30 day schedule, and does not automatically renew at the beginning of each month. If you make more than the 5 requests using the photoduplication from familysearch, you will get the following message:

"Thank you for your confidence in FamilySearch. As posted in FamilySearch/WIKI, there is a limit of five (5) requests per 30-day period. This request is in excess of that. Thank you, FamilySearch Support Photoduplication".

Please note, that the photoduplication runs on a 30 day period.

Photoduplication does not automatically renew at the beginning of each month!!!!

Make your requests wisely. Suggestion is to make your next request 30 days after submitting your 5th request. (5 images). One image per request. Not five requests for 25 images.

the two (2) New York City Death certificates.

You know you can just click the right name and exact date of some basics information in the mid 19th or 20th-century ancestor people and you will get your information easily in New York County death certificate here at any time.
